How to file an insurance claim for your Alpha-Stim® device
We offer insurance filing services for all models of Alpha-Stim® devices purchased directly from Allevia Health, Inc. Most private insurance plans cover the Alpha-Stim® 100 and Alpha-Stim® PPM, and make a partial payment, when these models are prescribed for some type of chronic pain in the body. For other diagnoses, and for the Alpha-Stim® SCS, a lower but significant percentage of claims are approved, often with a partial payment made.

To initiate a claim:

STEP 1: Please contact us to request our Insurance Claim Processing Patient Information Form.

STEP 2: FAX (a) this claim form, (b) your Statement of Medical Necessity, and (c) Rent-to-Own form to us at (888) 684-8414 or mail to Allevia Health, Inc. at the address below. Given that it may take up to 60 days to fully process a claim for an Alpha-Stim®, we suggest that you start with our rent-to-own plan (or call 800-684-9343 to purchase) while your claim is pending, and we will ship your Alpha-Stim® to you immediately by UPS.

STEP 3: If your insurance claim is approved, the payment received from your insurance company will be applied to the purchase of your Alpha-Stim®, or to the latter payments on your rent-to-own plan. If your claim is paid in full, then we will refund your overpayment.

Please note that while we can seek pre-approval for you and we can inquire about what your benefits cover, the information we receive from the insurance company is often not complete and not binding to them. Often patients do not know how much their insurance will cover until the claim has been filed. If you would prefer to file yourself for reimbursement from your insurance company, you may do so and we have billing codes and other information to help. (Call or e-mail us to request.).

What if my insurance does not cover the Alpha-Stim®?
For some patients the Alpha-Stim® is an out-of-pocket expense. For this reason we offer different rent-to-own options on all three Alpha-Stim® models to meet your needs. With our rental plans you may rent a new Alpha-Stim® device for as little as $82.50-$100/month (see the rental contract for details). You may return your rental unit at any time, with no further payments due, or if you continue renting, you will ultimately own your device at the end of the rental period. Please keep in mind that even if you pay for your Alpha-Stim® out-of-pocket, given its efficacy and safety, this technology is one of the most cost effective treatment options available for the treatment of anxiety, depression, insomnia and pain. It also comes with five year warranty.
