•	Ask your healthcare provider if Alpha-Stim® is appropriate for you. All rentals of Alpha-Stim® units require a prescription (Rx) or a statement of medical necessity.

•	Fill out one of our Alpha-Stim® rental agreement forms.

•	Sign and fax your rental agreement and Rx to 888-684-8414. If you prefer, you may scan both documents and send them to us at info@alleviahealth.com as email attachments, or you may mail them to: Allevia Health, Inc. 2175 NW Professional Drive, Suite M Corvallis, OR 97330.
•	Once we receive your Rx and a signed rental agreement, an Alpha-Stim® unit will be mailed to you (FREE) by USPS Priority Mail to the address specified in the rental agreement.
•	We will be available by phone to help you learn how to use your Alpha-Stim® device. Your rental plan includes free technical support.
•	Your credit card will be billed a low monthly fee for each month of rental.
•	You may terminate the rental agreement by returning the unit at any time or you may continue paying monthly rental fees until the balance is fully paid off. All rental expenses are applied to the purchase price of your own device.

Rentals of Alpha-Stim® devices from Allevia Health, Inc. available to US residents only. Requires a major credit card account and a damage deposit.
